Permission forms for all 16 year old donors must be signed by a parent or guardian.

In order to donate blood, 16 year old donors must have:

  • 1 Form of Identification Below:

    -Government Issued ID
    State ID, Driver's License, Military ID, Passport

    -Certified Birth Certificate

    -School or Work Photo ID

    -Class Schedule with Date of Birth

Basic donor guidelines:

  • At least 16 years of age on the date of the blood drive.
    •
  • At least 110 pounds.
    •
  • Eat a well balanced meal, including plenty of fluids (avoid caffeine), 3-4 hours prior to your donation.
